    We can not register any players without a parent being linked to them.

  • Max. file size: 256 MB.
    This should be a passport style photo which is only head and shoulders. This photo will also be used for league registration.

    If you do not upload one here, you will need to send one to Ellen ASAP in order to be registered for the team

  • Please detail here any medication, allergies or conditions that we should know about.
    In the event that your child is injured whilst playing football/travelling to and from football events and you cannot be contacted, do you give consent for your child to receive medical attention?


  • Permissions For Players

    Photographs may be taken at any LFS event for marketing purposes. Do we have permission to take photos of your child?
    IF YOU SELECT NO IT WILL MEAN WE CAN NOT FILM ANY SESSION YOUR CHILD IS IN.
    Videos may be taken at any LFS event for coaches and player development. These videos may be used for promotions by us
    We believe it is our responsibility to ask you to ensure the safety of the attendees at LFS. Do you agree to ensure your child comes equipped to their session with appropriate footwear (moulds or studs), warm clothing, shin pads and adequate drink for the duration of the session as well as any medication they may require.
